The Application for Historical Vessel Plaque form (BOAT 100) is used by individuals to request a special plaque identifying their craft as a vessel of historical interest. To qualify for this designation the vessel must have been constructed of wood prior to December 31, 1942.
A one-time fee of $20.00 is charged for the plaque. Forms and fees can be sent to the department at:
