You need a license to operate as an all-terrain vehicle (ATV) safety training organization or instructor.
ATV Safety Training Organizations
How to apply for an ATV safety training organization license
Apply for your license and pay your fees online.
To apply for your license, submit the following:
- Application for Occupational License Personal History Questionnaire, Business Licensing Unit (OL 29B).
- All-Terrain Vehicle Safety Training Organization Insurance Certificate (OL 133).
- Application for Occupational License (OL 216).
- Driving School Owner or All-Terrain Vehicle Safety Training Organization Principal Surety Bond (OL 218).
- Live scan fingerprint and Request for Live Scan Clearance (receipt) (DMV 8016) for each person listed on the OL 216.
- A copy of articles of incorporation, corporate minutes, or other document filed with the Secretary of State, which identifies the officers, shareholders, and managers with 10 percent or more interest in the business (corporation, limited liability company, or limited liability partnership owned businesses only).
- Copy of your fictitious name statement.
You have three attempts to pass the written exam.
Your application and fees are valid for one year. If you do not complete your application within the year, you will need to reapply, repay the fees, and retake the exam.
You will also need to undergo a background investigation.
Fees
- $150 non-refundable application fee
- $1 for family support program
- $42 per owner if you submit fingerprint card(s) in-lieu of live scan receipt
- $70 for each branch license or license modification, or change in corporate structure
How to renew an ATV safety training organization license
Renew your license and pay your fees online.
To renew your license, complete a Driving School Owner, Traffic Violator School Owner, and All-Terrain Vehicle Safety Training Organization Renewal Application (OL 45A) and mail it to the address on the form.

- $50 renewal application fee
- $1 for family support program

Modify your license and pay your fees online.
To modify an existing license, complete an Application for Modification to a Driving School Owner License and All-Terrain Vehicle Safety Training Organization (OL 226) and mail it to:

How to apply for an ATV safety training instructor license
Apply for your license and pay your fees online.
To apply for your license, submit the following:
- Application for Driver Instructor and All-Terrain Vehicle Safety Instructor License (OL 16I).
- Live scan fingerprint and Request for Live Scan Clearance (receipt) (DMV 8016).
- Fingerprint card (ADM 1316) (out-of-state applicants only). Fingerprint cards may be obtained by calling Occupational Licensing (OL) at (916) 229-3126
- Appointment of Director as Agent for Service of Process (ADM 9050).
- Certificate from ATV safety association confirming a passing score for an instructor written test and skill test.
- Proof of high school graduation or equivalent.
