How to apply for an ATV safety training organization license
Apply for, renew, or modify your license and pay your fees online. How-to videos and a quick start guide are available. If you experience technical issues or get locked out of your account, see technical support.
When applying, have the following items ready for upload:
- Business ownership structure information, financial institute, property, and surety bond information.
- Business address, hours of operation, and modality.
- Secretary of State (SOS) Statement of Information (Corporation, Limited Liability Company, Partnership/Limited Partnership).
- Completed Request for Live Scan Clearance (receipt) (DMV 8016) as proof of completing a Live Scan fingerprint for each person listed as owner.
- Personal identifying information, background information, and conviction history.
- If “doing business as”, a copy of the fictious name statement is required.
- Completed Appointment of Director as Agent for Service of Process (ADM 9050).
Application and fees are valid for one year from submission date. If the application is not completed within the year, new application and fees will be required.

How to apply for an ATV safety training instructor license
Apply for, renew, or modify your license and pay your fees online. How-to videos and a quick start guide are available. If you experience technical issues or get locked out of your account, see technical support.
When applying, have the following items ready for upload:
- Personal identifying information, employment, education, and conviction history.
- Proof of age (must be at least 18 years old).
- Completed Request for Live Scan Clearance (receipt) (DMV 8016) as proof of completing a Live Scan fingerprint.
- Certificate confirming a passing score for instructor exam proctored by a licensed ATV safety organization.
- Proof of high school graduation or equivalent.
Application and fees are valid for one year from submission date. If the application is not completed within the year, a new application, fees, and exam will be required.
