You need a traffic violator school (TVS) owner license if you own a TVS that offers traffic safety instruction for individuals referred by a court. A TVS must have a licensed owner, operator, and instructor to be authorized to give instruction.
How to Apply for a TVS Owner License
All occupational license applicants must create an online account and apply online – how-to videos and a quick start guide are available. If you experience technical issues or get locked out of your account, see technical support.
Have the following information and supporting documents ready for upload:
- Proposed business name, business address, and owner information.
- Business ownership structure, financial institute, location, and surety bond information.
- Property ownership or rental agreement document, hours of operation, and modality.
- Personal identifying information, employment, education, and conviction history.
- Secretary of State (SOS) Statement of Information (Corporation, Limited Liability Company, Partnership/Limited Partnership).
- If “doing business as,” a copy of the fictious name statement is required.
- Lesson plan, original or purchased. If utilizing a purchased lesson plan, the lesson owner’s information and a letter of authorization is needed.
- Completed Request for Live Scan Clearance (receipt) (DMV 8016) (PDF) for each person listed as owner (original application only). Out-of-state applicants must mail a completed Fingerprint Card (ADM 1316). Contact DMV to request an ADM 1316.
- Completed Traffic Violator School Classroom Lease or Rental Agreement (OL 144) (PDF) if not located at the business address.
- Completed Appointment of Director as Agent for Service of Process (ADM 9050) (PDF).
- Completed Property Use Verification for a Driving School or Traffic Violator School License (OL 140).
- Photos of business office, signage, and location (refer to OL photography requirements).
Application and fees are valid for one year from submission date. If the application is not completed within the year, new application and fees will be required.
Fees: TVS Owner License
Note When applying for a combination of owner/operator or owner/instructor license, only the owner fee will be due.
