REG 4017

Permanent Trailer Identification Certification (REG 4017)

The REG 4017 is a two-sided form used for trailers in the PTI program. Side A is completed and signed by sole owners requesting a paper certificate of title. Side B is an application for: the transfer of ownership for a PTI trailer with an electronic title record or addition of a legal owner only. The transfer may be a registered owner transfer or a release of security interest only.

Statement of Personal History – Pre-Implementation Screening Process (REG 4019)

This form is required to determine the eligibility of the employees who will participate in the Business Partner Automation Program. The form must be completed by business owners, managers, and administrative staff responsible for the oversight of the program or who manage or administer transactions, inventory, employees, or money; order or account for inventory; and employees who process vehicle registration transactions or work directly with customers.

SR 19C

Financial Responsibility Information Request (SR 19C)

Each driver involved in an accident that caused more than $1,000 ($750 for accidents occurring prior to January 1, 2017) worth of property damage or injured or killed any person must complete a Traffic Accident Report form (SR 1) within 10 days. You may obtain a photocopy of the report that the other driver submitted to DMV by completing a Financial Responsibility Information Request (SR 19C) form and mailing it to the address printed on top of the form.
