Have the following information and supporting documents ready to upload:
- Business ownership structure, location, and modality information.
- Personal identifying information.
- A copy of your lesson plan.
- Secretary of State (SOS) Statement of Information (Corporation, Limited Liability Company, Partnership/Limited Partnership).
