1.095 Name Statement Requirement (VC §1652)
The Name Statement section of a Statement of Facts (REG 256) (PDF) form must be completed when an applicant’s printed name and signature are so different that it appears to be two different people. A name statement is not required for signatures containing less than the printed name.
